Submittal of this form formalizes a request for funding support for a field trip to Frost Science for Title I schools. Approval is subject to availability of funding and is on a first come, first served basis. Completion of this form does not guarantee funding. Community access funds cover general field trip admission for students, but do not cover transportation, Learning Labs, or Planetarium shows.

If approved, an invoice confirmation will be emailed to the person submitting this form within 5 business days. Please note, that a letter or email from your principal stating that the field trip has been approved and a signed confirmation document is required to hold your date
