How to Enter this Sweepstakes:
This Sweepstakes is open to individuals age 18 or older who receive the COVID-19 vaccination at the Frost Science COVID-19 vaccination site. You may enter the Sweepstakes by visiting your COVID-19 vaccination at Frost Science on either June 11, 2021-June 13, 2021 between the hours of 10:00am-6:00pm each day and completing a Sweepstakes entry form. Entry forms will be available at the Frost Science vaccination site registration table. A ticket to Frost Science is not required to receive and submit an entry form. All entries are due by 6:00pm on June 13, 2021. No mechanically reproduced entries will be accepted.
Entry Form:
Participants must complete the entry form and provide a valid e-mail address. Sponsor is not responsible for any entry errors or failures, regardless of whether they are human or technical in nature. Sponsor is also not responsible for incorrect or inaccurate entry of information, whether caused by Internet users, or by any of the equipment or programming associated with or utilized in the Sweepstakes, or by any technical or human error that may occur in the processing of the entries in the Sweepstakes. Sponsor is not responsible for any lost or misdelivered entries. In the event of a dispute regarding the individual who actually submitted an entry, the entry will be deemed submitted by the name on the entry form.
All requested information must be completed to enter. No purchase is necessary to claim a prize. Limit (a) one Sweepstakes entry form per person. By entering, all participants agree to be bound by the official rules of these Sweepstakes. Eligibility:
This Sweepstakes is open only to individuals who, at the time of entry, are (i) legal U.S. residents located in the U.S., and (ii) age 18 and older. Employees, officers, and trustees of Museum of Science, Inc., and its onsite vendors, and advertising and promotion agencies, as well as the immediate family members and/or persons living in the same households of each, are not eligible for this Sweepstakes. This Sweepstakes is void where prohibited.
Drawings:
Winner will be selected in a random drawing conducted on or about June 16, 2021 from among all eligible entries received. Judges’ decisions are final and binding in all matters relating to this Sweepstakes. ODDS OF WINNING ARE BASED UPON THE NUMBER OF ELIGIBLE ENTRIES RECEIVED. Winner will be notified by e-mail.

Waiver, Release, and Indemnity::

Participants further acknowledge that said parties have neither made nor are in any manner responsible or liable for any warranty, representation, or guarantee expressed or implied, in fact or in law, relative to any prize, including, but not limited to, its quality or fitness for a particular purpose.
Publicity Release:
By accepting a prize, each winner agrees to the use of his/her name, likeness, voice, and/or biographical information by Sponsor, or any party authorized by Sponsor, for advertising and promotional purposes, without additional compensation, except where prohibited by law. Entries become the property of the Sponsor and will not be returned.
Applicable Law; Arbitration:
This Sweepstakes is governed by U.S. law and is subject to all applicable federal, state and local laws and regulations. Void where prohibited by law. All issues and questions concerning the construction, validity, interpretation and enforceability of these Official Rules, or the rights and obligations of entrant and Sponsor in connection with this Sweepstakes, shall be governed by, and construed in accordance with, the laws of the State of Florida without giving effect to the conflict of laws, and any matters or proceedings which are not subject to arbitration as set forth in these rule and/or for entering any judgment on an arbitration award, shall take place in the State of Florida, in the City of Miami.
The parties waive all rights to trial in any action or proceeding instituted in connection with these Official Rules and/or this Sweepstakes. Any controversy or claim arising out of or relating to these Official Rules and/or this Sweepstakes shall be settled by binding arbitration in accordance with the commercial arbitration rules of the American Arbitration Association. Any such controversy or claim shall be arbitrated on an individual basis and shall not be consolidated in any arbitration with any claim or controversy of any other party. The arbitration shall be conducted in the State of Florida, in the City of Miami, and judgment on the arbitration award may be entered into any court having jurisdiction thereof.
If any provisions of these Official Rules are determined by a court of competent jurisdiction to be invalid or unenforceable, such determination shall not affect the validity or enforceability of any other provision.
General:
All taxes and any other fees or costs associated with the Prizes are the sole responsibility of the winner. The Prize is nontransferable, and no substitutions or cash redemptions will be made, except as provided herein. In the event the Prize is unavailable, Sponsor reserves the right to award a prize of equal or greater value. The winners will be required to sign and return an affidavit of eligibility and a liability and publicity release within 5 calendar days of the date when such documents are sent to winner, or prize will be forfeited and awarded to an alternate winner. The Prizes will be awarded to verified winners.
In the event of noncompliance with these Official Rules, if a winner cannot be reached at the e-mail address in winner’s entry form within 5 calendar days of the first attempt, or if the prize or prize notification is returned as unclaimed or undeliverable, the particular prize will be forfeited, and an alternate winner will be selected.
Sponsor is not responsible for any damage to a participant’s system that is occasioned by participating in this Sweepstakes or by downloading any information necessary to participate in the promotion.
Sponsor assumes no responsibility for any error, omission, interruption, deletion, theft, or destruction, or unauthorized access to, or alteration of, entries. Entries that have been tampered with are void. Sponsor reserves the right to disqualify any participant who Sponsor determines, in its sole discretion, has threatened the operation of this Sweepstakes or website, or violates these Official Rules. Sponsor reserves the right, in its sole discretion, to cancel or suspend this Sweepstakes should viruses, bugs, or other causes beyond Sponsor’s control corrupt the administration, security, or proper play of this Sweepstakes.
